About this test

The serum aquaporin-4 antibody (NMO-IgG) test detects autoantibodies against aquaporin-4, a water channel protein in the brain, spinal cord, and optic nerves. Serum is the preferred sample, as it is more sensitive than CSF.

A normal result is negative.

A positive result confirms neuromyelitis optica spectrum disorder (NMOSD), which causes optic neuritis, transverse myelitis, and intractable hiccups or vomiting.

The test helps distinguish NMOSD from multiple sclerosis (MS), guiding the correct treatment. If the result is negative, MOG antibody testing may be considered.
