What does a sperm DNA fragmentation test measure?

It measures breaks in the DNA that sperm carry, reported as the DNA Fragmentation Index (DFI): the share of sperm with fragmented DNA. We use the sperm chromatin dispersion (SCD) method, one of the methods described in the WHO semen manual. Under the microscope, sperm with intact DNA form a halo; sperm with fragmented DNA form a small halo or none.

What is a normal DFI?

My semen analysis was normal. Do I still need this test?

It can still be worth it. A semen analysis counts sperm and checks how they move and look, but not the DNA inside them. The WHO semen manual notes that DNA fragmentation is only partly related to semen quality and has been linked to infertility in men with normal semen results. Guidelines recommend the test after repeated miscarriages, failed IUI or IVF, or infertility with no clear cause.

Who should get a sperm DNA fragmentation test?

European urology guidelines strongly recommend it for couples with recurrent pregnancy loss or failed assisted reproduction such as IUI or IVF, and for men with unexplained infertility. It is also worth discussing with your doctor if you have a varicocele, smoke, are overweight, are often exposed to heat, are over 40, or have had a genital infection or chemotherapy.

How do I prepare for the test?

Avoid ejaculation for 2–7 days before your visit: not less than 2 days and not more than 7. Avoid alcohol, smoking, hot tubs and saunas, drink enough water and sleep well the night before. Bring a valid ID, your doctor's prescription and any old reports. If you have had a fever, an infection or COVID in the last 3 months, talk to us before you book; we may advise rescheduling.
