About this test

The iron stain test, also called Perls' Prussian blue stain, detects stored iron (hemosiderin) in bone marrow, blood, urine, or tissue samples such as the liver.

In bone marrow, absent iron confirms iron deficiency anemia, while ring sideroblasts suggest sideroblastic anemia or MDS. In liver biopsies, excess iron indicates hemochromatosis or iron overload. Urine hemosiderin points to intravascular hemolysis, as seen in PNH.

This test helps diagnose iron-related disorders when routine blood tests are inconclusive.

Questions about this test

When should I get the Iron Stain test?

An iron stain test is usually advised by a doctor when anemia remains unexplained or routine iron tests are inconclusive. It is recommended when sideroblastic anemia, myelodysplastic syndrome (MDS), hemochromatosis, iron overload, or intravascular hemolysis is suspected. It is often performed during a bone marrow or liver biopsy.

How should I prepare for the Iron Stain test?

Preparation for an iron stain test depends on the sample type. For a bone marrow or liver biopsy, inform your doctor about blood thinners, allergies, bleeding disorders, or pregnancy, and you may need to fast if sedation is planned. For a urine sample, collect a clean first-morning specimen. Follow your doctor's instructions.

Do I require fasting for the Iron Stain test?

Fasting is usually not required for an iron stain test on urine or blood samples. However, if the test is done on a bone marrow or liver biopsy under sedation, you may need to fast for 4–6 hours beforehand. Always follow your doctor's or hospital's specific instructions.
