Free Testosterone

You should get a free testosterone test if men have low libido, erectile dysfunction, fatigue, reduced muscle mass, or infertility, especially when total testosterone is borderline. Women may need it for excess facial or body hair, acne, hair thinning, or irregular periods suggestive of PCOS. It also helps monitor testosterone therapy.

Fasting for 8–12 hours is often recommended before a free testosterone test, as eating can lower testosterone levels. Give your sample in the morning (around 7–10 AM), when levels peak. Women should ideally test on days 2–5 of the menstrual cycle. Inform your doctor about testosterone therapy, steroids, or hormonal medicines, and avoid testing during acute illness.
