The 17-hydroxyprogesterone (17-OHP) test measures a hormone produced by the adrenal glands and ovaries during cortisol production.
Normal 17-OHP levels vary by age, sex, and menstrual phase, so results are interpreted using lab-specific ranges.
High 17-OHP is the key marker of congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency. In newborns, it is used for screening. In women, mildly raised levels suggest non-classic CAH, which causes excess hair growth, acne, irregular periods, and infertility, often mimicking PCOS.
Borderline results may need an ACTH stimulation test.
You should get a 17-hydroxyprogesterone test if you have excess facial or body hair, severe acne, irregular periods, or infertility, especially when PCOS is suspected. It is also used for newborn screening for congenital adrenal hyperplasia (CAH), for babies with ambiguous genitalia, for early puberty in children, and for monitoring CAH treatment.
Fasting is usually not required for a 17-hydroxyprogesterone test, but give your sample early in the morning (around 7–9 AM), when levels are most reliable. Women should test during the early follicular phase (days 2–5 of the menstrual cycle). Inform your doctor about steroids, hormonal medicines, oral contraceptives, or pregnancy.
