About this test

The high-sensitivity troponin I (hs-TnI) test measures very small amounts of cardiac troponin I, a protein released when heart muscle is damaged. It is highly specific to the heart.

Normal hs-TnI levels are typically below about 20–30 ng/L, varying by assay and sex.

Its high sensitivity allows earlier detection of a heart attack, often within 1–3 hours of symptom onset, enabling rapid diagnosis or rule-out with serial testing. Raised levels may also occur in myocarditis, heart failure, pulmonary embolism, and sepsis.

Questions about this test

When should I get the Troponin I (High Sensitive) test?

A high-sensitivity troponin I test is done urgently if you have chest pain or pressure, pain spreading to the arm, jaw, or back, shortness of breath, sweating, nausea, or dizziness, the warning signs of a heart attack. If you experience these symptoms, seek emergency medical care immediately. It is also used to monitor heart muscle damage.

How should I prepare for the Troponin I (High Sensitive) test?

No special preparation or fasting is required for a high-sensitivity troponin I test. It is often done in an emergency, so never delay testing if you have chest pain. Inform your doctor about kidney disease, recent strenuous exercise, or biotin supplements, as these may affect results. Repeat tests are usually done 1–3 hours apart.

Do I require fasting for the Troponin I (High Sensitive) test?

No, fasting is not required for a high-sensitivity troponin I test. It can be done at any time, regardless of meals, and is often performed in an emergency. If you have chest pain or other heart attack symptoms, don't delay; seek medical care immediately. Repeat tests may be needed 1–3 hours apart.
