The PSA test measures prostate-specific antigen, a protein produced by the prostate gland. It is the main blood test for prostate cancer screening.
Normal PSA levels are generally below 4 ng/mL, though normal values increase with age.
PSA levels between 4 and 10 ng/mL are borderline and may need a free PSA test. Levels above 10 ng/mL increase the likelihood of prostate cancer. Raised PSA can also occur with benign prostate enlargement (BPH), prostatitis, urinary infections, recent ejaculation, or cycling.
PSA is also used to monitor prostate cancer treatment and recurrence.
You should discuss a PSA test with your doctor from age 50, or from age 40–45 if you have a family history of prostate cancer. It is also recommended if you have urinary symptoms like frequent urination, weak flow, or difficulty urinating, blood in the urine, or pelvic pain, and for monitoring prostate cancer treatment.
No fasting is required for a PSA test. Avoid ejaculation and vigorous exercise, especially cycling, for 48 hours beforehand. Give your sample before a digital rectal exam or prostate procedure. Inform your doctor about any urinary infection, recent prostate biopsy, or medicines like finasteride or dutasteride, as these can affect PSA results.
No, fasting is not required for a PSA test. You can eat and drink normally before giving your sample. However, avoid ejaculation and vigorous exercise, especially cycling, for 48 hours beforehand, as these can temporarily raise PSA levels. Give your sample before any prostate examination or procedure.
