Progesterone

The progesterone test measures progesterone, a hormone produced mainly by the ovaries after ovulation and by the placenta during pregnancy. It prepares the uterus for and supports pregnancy.
Normal progesterone levels vary by menstrual cycle phase and pregnancy stage. A mid-luteal (day 21) level above 3 ng/mL confirms ovulation.
Low progesterone suggests anovulation, PCOS, a luteal phase defect, or a higher risk of miscarriage or ectopic pregnancy. High progesterone may occur in pregnancy, ovarian cysts, congenital adrenal hyperplasia, or with progesterone therapy.
You should get a progesterone test if you have difficulty conceiving, irregular periods, or recurrent miscarriages, to check whether you are ovulating. It is also recommended in early pregnancy with bleeding or abdominal pain, to assess miscarriage or ectopic pregnancy risk, and for monitoring fertility treatment or progesterone therapy.
No fasting is required for a progesterone test. To check ovulation, give your sample on day 21 of a 28-day cycle, or about 7 days before your expected period if your cycle is longer or shorter. Inform your doctor about oral contraceptives, hormone therapy, fertility medicines, or pregnancy. Stop biotin supplements 48–72 hours beforehand.
No, fasting is not required for a progesterone test. You can eat and drink normally before giving your sample. However, timing matters: to check ovulation, the test is usually done on day 21 of a 28-day cycle, or about 7 days before your expected period. Stop biotin supplements 48–72 hours beforehand.
