The Free T3 (FT3) test measures the unbound, biologically active form of triiodothyronine, the most potent thyroid hormone. It regulates metabolism, heart rate, and energy levels.
Normal Free T3 levels are about 2.3–4.2 pg/mL, varying by lab.
High FT3 indicates hyperthyroidism, such as Graves' disease or a toxic nodule, and helps detect T3 toxicosis, where FT3 is high but FT4 is normal. Low FT3 may suggest hypothyroidism, severe illness (sick euthyroid syndrome), or starvation.
FT3 is interpreted with TSH and FT4 to diagnose and monitor thyroid disorders.
You should get a Free T3 test if you have symptoms of hyperthyroidism, such as weight loss, palpitations, anxiety, sweating, or tremors, especially when TSH is low but FT4 is normal. It is also recommended for evaluating hypothyroidism, monitoring Graves' disease or anti-thyroid treatment, and assessing thyroid function during serious illness.
No special preparation or fasting is required for a Free T3 test. Give your sample in the morning, and take your thyroid medicine after the sample unless your doctor advises otherwise. Stop biotin supplements 48–72 hours before the test, as biotin can interfere with results. Inform your doctor about pregnancy or other medications.
