About this test

The free PSA test measures prostate-specific antigen (PSA) that circulates unbound in the blood. It is interpreted as the percentage of free PSA relative to total PSA.

It is most useful when total PSA is borderline (4–10 ng/mL).

Free PSA percentage interpretation:

  • Above 25%: lower risk, more likely benign prostate enlargement (BPH)
  • 10–25%: intermediate risk
  • Below 10%: higher risk of prostate cancer

The test helps distinguish prostate cancer from BPH and reduces unnecessary biopsies.

Questions about this test

When should I get the Free PSA test?

You should get a free PSA test if your total PSA is mildly raised (usually 4–10 ng/mL) or borderline, to help decide whether a prostate biopsy is needed. It is also recommended for men with urinary symptoms, an enlarged prostate, or a family history of prostate cancer, as advised by a doctor.

How should I prepare for the Free PSA test?

No fasting is required for a free PSA test. Avoid ejaculation and vigorous exercise, especially cycling, for 48 hours beforehand. Give your sample before a digital rectal exam or prostate procedure. Inform your doctor about any urinary infection, recent prostate biopsy, or medicines like finasteride or dutasteride, as these can affect results.

Do I require fasting for the Free PSA test?

No, fasting is not required for a free PSA test. You can eat and drink normally before giving your sample. However, avoid ejaculation and vigorous exercise, especially cycling, for 48 hours beforehand, as these can temporarily raise PSA levels. Give your sample before any prostate examination or procedure.
