Apolipoproteins E

The Apo E test evaluates apolipoprotein E, a protein that helps transport and clear cholesterol and triglycerides from the blood. It is often done as genotyping to identify the E2, E3, or E4 variants.
E3 is the most common, neutral type. E4 increases the risk of high LDL cholesterol, heart disease, and late-onset Alzheimer's disease. E2/E2 is linked to familial dysbetalipoproteinemia (type III hyperlipoproteinemia).
The test helps evaluate unexplained lipid disorders and inherited cardiovascular risk.
You should get an Apo E test if you have very high cholesterol and triglycerides that don't respond to treatment, yellowish skin deposits (xanthomas), or a family history of early heart disease. It may also be considered for assessing Alzheimer's disease risk, ideally with genetic counseling, as results don't confirm the disease.
Fasting is not required for an Apo E genotyping test, as it analyzes your DNA, which doesn't change with meals. However, if it is done with a lipid profile, fast for 10–12 hours, drinking only water. Consider genetic counseling before testing, especially if you are testing for Alzheimer's risk.
No, fasting is not required for an Apo E genotyping test, because it analyzes your DNA, which is not affected by food. However, if your doctor has ordered a lipid profile or Apo E level alongside it, you may need to fast for 10–12 hours, drinking only water. Follow your lab's instructions.
