APO B

The Apo B test measures apolipoprotein B, the main protein in LDL, VLDL, and other "bad" cholesterol particles. Each harmful particle carries one Apo B molecule, making it a better predictor of heart risk than LDL alone.
Desirable Apo B levels are below about 90 mg/dL. Levels above 130 mg/dL are considered high.
High Apo B increases the risk of heart attack, stroke, and atherosclerosis, and is seen in familial hypercholesterolemia, diabetes, metabolic syndrome, and hypothyroidism.
The Apo B/Apo A1 ratio helps assess cardiovascular risk and monitor statin therapy.
You should get an Apo B test if you have a family history of early heart disease, high triglycerides, or risk factors like diabetes, obesity, metabolic syndrome, or high blood pressure. It is especially useful when LDL looks normal but heart risk seems high, and for monitoring statins or other lipid-lowering treatment.
Fasting is not strictly required for an Apo B test, as meals have little effect on Apo B levels. However, if it is done with a lipid profile, fast for 10–12 hours, drinking only water. Avoid alcohol and fatty foods for 24 hours beforehand, and inform your doctor about any cholesterol-lowering medicines or hormone therapy.
Fasting is usually not required for an Apo B test alone, because meals have little effect on Apo B levels. However, if your doctor has ordered a lipid profile alongside it, you should fast for 10–12 hours, drinking only water. Follow your doctor's or lab's specific instructions.
