The Apo A1 test measures apolipoprotein A1, the main protein in HDL (good cholesterol). It helps remove excess cholesterol from the blood and protects against heart disease.
Normal Apo A1 levels are roughly 110–200 mg/dL, varying by sex and lab.
Low Apo A1 increases the risk of heart attack, stroke, and atherosclerosis. It is linked to smoking, obesity, diabetes, kidney or liver disease, and rare genetic disorders like Tangier disease.
The Apo B/Apo A1 ratio is a strong predictor of cardiovascular risk.
You should get an Apo A1 test if you have low HDL cholesterol, a family history of early heart disease, or risk factors like diabetes, obesity, high blood pressure, or smoking. It is also recommended when routine lipid results don't explain your heart risk, and for monitoring lipid-lowering treatment alongside Apo B.
Fasting for 10–12 hours is usually recommended before an Apo A1 test, especially if it is done with a lipid profile. You may drink water during this time. Avoid alcohol and fatty foods for 24 hours beforehand, and inform your doctor about any cholesterol-lowering medicines, hormone therapy, or oral contraceptives.
Yes, fasting is usually recommended for an Apo A1 test. You should fast for 10–12 hours before giving your sample, though drinking water is allowed. This is especially important when the test is done with a lipid profile. Avoid alcohol for 24 hours beforehand, and follow your lab's specific instructions.
