About this test

The 24-hour urine porphobilinogen (PBG) test measures PBG, a substance formed during heme production, excreted in urine over a full day.

Normal PBG levels are typically below 2 mg/24 hours, varying by lab.

Markedly high PBG is the key marker of acute hepatic porphyrias, especially acute intermittent porphyria (AIP). These cause attacks of severe abdominal pain, vomiting, constipation, dark or reddish urine, muscle weakness, confusion, and seizures, often triggered by certain medicines, alcohol, fasting, or hormonal changes.

Testing is most accurate during an attack, and the sample must be protected from light.

Questions about this test

When should I get the 24 HOUR URINE PORPHOBILINOGEN-PBG,WADAJ test?

You should get a 24-hour urine porphobilinogen (PBG) test if you have unexplained episodes of severe abdominal pain, vomiting, constipation, dark or reddish urine, muscle weakness, confusion, or seizures, especially after certain medicines, alcohol, or fasting. It is also recommended for a family history of porphyria and for monitoring known acute porphyria.

How should I prepare for the 24 HOUR URINE PORPHOBILINOGEN-PBG,WADAJ test?

To prepare for a 24-hour urine PBG test, discard your first morning urine and note the time. Then collect all urine for the next 24 hours, keeping the container refrigerated and protected from light (wrapped in foil or a dark bag). Ideally, collect during or soon after an attack, before treatment. Avoid alcohol, and inform your doctor about all medicines.

Do I require fasting for the 24 HOUR URINE PORPHOBILINOGEN-PBG,WADAJ test?

No, fasting is not required for a 24-hour urine porphobilinogen (PBG) test. In fact, you should eat regular meals, as fasting can trigger porphyria attacks. Avoid alcohol, keep the urine container refrigerated and protected from light, and collect every urine sample over the full 24 hours.
